Onboarding Documents

Welcome New Staff Candidate;

Below are 4 links to forms that must be completed in order for us to assign you to an employment position. Please fill out the required fields and digitally sign each document where indicated so that we may enter you into our system. Any field marked with an (* ) asterisk is required.

(HINT: at the top of the form it will indicate how many required fields have been completed and how many remain to be completed). When you complete all required info in any document click the “FINISH” button and then proceed to the next document.

If the “FINISH” button reports that all required fields have not been completed, click the “next” button to reveal any missing required fields.

If you close your browser with the”FINISH” button reporting that some required fields still remain to be filled, the form will be incomplete and then all of your work will be lost as the form will not process.

• USCIS form I-9:  Page 2 of form I-9 shows a list of identification and Employment Authorization documents. Please upload the required documents using the buttons on page 2. If you are unable to use the upload feature click the checkbox at the bottom of page 2 to text the images to us.
